Please complete all blanks on the application.

Degree:  Check the box corresponding to the degree you are applying to receive.  Be careful to select the correct application for your degree; there are two different Bachelor’s applications.  Note: Applying to receive both a Bachelor's degree and an Associate's degree at the same commencement requires two separate applications.

Completion date: Indicate the month and year you expect to complete your academic requirements (no later than the last day of the term prior to the graduation date).  Indicate at which Wayland campus you will complete your final courses.

Name on diploma: Legibly print or type your name as you want it on your diploma.  Please use upper and lower case.  Acceptable punctuation includes commas, periods, hyphens, and recognized pronunciation marks.  Quotation marks, unusual characters, super- or sub-scripted letters, all capital letters, titles (Mr., Mrs., etc) and punctuation other than previously listed are not allowed.

My hometown for the graduation program: Not used by the San Antonio campus.  Only your name and degree will be listed on the program.

Social Security Number/Student ID Number: Your ID number can be found on your Wayland ID card or on your registration paperwork.  Include all nine digits, including leading zeros.  You are not required to list your SSN if you have listed your ID Number.

Current mailing address/phone numbers/e-mail address:  If the information on the application is different than what we have in our database, a Change of Student Information request will be generated.  This is the address to which we will send all graduation correspondence, unless you have provided other instructions in writing.

Permanent address: Indicate "same as above" or your permanent address.

Graduation gown sizing: Indicate height in feet and inches (fractions will be rounded up to the next inch), approximate weight, and gender.  This information is used to calculate gown sizing and is distributed on a "need to know" basis.  Please include this information even if you are not planning to attend the ceremony.  If you are pregnant, include your current weight and how far along you will be as of the date of the commencement exercises.

Optional Instructions to Campus for Diploma: Not used by the San Antonio campus.  If you will not be attending commencement exercises, we require a completed Request to Graduate in Absentia form.

Military status as of the date of graduation: We are required to submit an official transcript to the military base at which you are stationed if Randolph AFB, Lackland AFB, or Ft. Sam Houston.  If your status changes after completing the Application, please contact the Graduation Secretary to have the change made to your record.

Check boxes:  Check whether you are applying or re-applying for graduation.  The campus is "San Antonio" and the graduation date will be either "February" or "June." 

Payment: Refer to the Payment Schedule at the bottom of the form for a break-down of the application fees.

Ceremony attendance:  Check whether you will or will not be attending the commencement ceremony.  If left blank, we will assume you are attending.  If you will not be attending, please complete a Request to Graduate in Absentia form and return it to the San Antonio campus no later than eight (8) days before the ceremony.

Signature and date:  An original signature and date is required to process the application.

Returning the form:  Faxed forms will not be accepted.  You may drop off the form along with you payment to the Front Office at the IH 35 North location or mail the form with a check or money order to:
